The membership form must be filled out by all persons
applying for the first time for membership.

All IASSID members renewing their membership
 are also requested to fill out this form.

In both cases after filling out this form you will
be automatically directed to the payment page.

All fields that are indicated with an * are mandatory fields that must be filled in prior to submitting a request.
